Transforming the Safety Net through Mobile Health

 

As health systems adopt more patient-centered and primary care-oriented models under health care reforms, improved understanding among providers as to how mobile technology may be used effectively will help them to meet the goals of the Triple Aim through improving their ability to provide greater connectivity between patients and their care providers, improving patient experiences with care, and activating and engaging patients in managing their own care. Through a grant from the Commonwealth Fund, CITPH will conduct the first national-level assessment of current knowledge and the state of practice related to the deployment of mobile health within the safety net, provide evidence as to which interventions have been most effective in improving care access and health outcomes, and reducing costs for vulnerable populations, and identify use cases that illustrate best practices or effective strategies for the deployment of mHealth solutions at scale.
